State Program Manager, Louisiana (Part-Time)

The State Program Manager (SPM) is responsible for driving the expansion of the Centering model at healthcare practices within their state through exceptional customer service and engagement with key stakeholders in the provider and payer communities. This part-time position focuses on building relationships and providing technical expertise. Due to the local nature of this work we are seeking candidates located in Louisiana.

Group Care Organizational Development Consultant

The Centering Consultant is both expert in the clinical implementation of the Centering model and a skilled facilitation trainer. They are equally comfortable engaging with clinical staff and leadership to ensure that the systems changes required to meet CHI’s quality and sustainability expectations are achieved. They are skilled facilitators with the ability to transfer those skills to providers and clinical care teams tasked with providing Centering care.

Practice Services Manager

The Practice Services Manager (PSM) manages the client relationship through its lifecycle and is the primary point of contact for all Centering practice sites within a geographic region. The PSM assesses site needs, coordinates the delivery of products and services necessary to implement Centering group care as well as provides regular outreach and support to ensure that sites are reaching their goals. The position is customer-service driven with a focus on anticipating site needs and identifying solutions to common challenges.
