State Program Manager, Louisiana (Part-Time)

The State Program Manager (SPM) is responsible for driving the expansion of the Centering model at healthcare practices within their state through exceptional customer service and engagement with key stakeholders in the provider and payer communities. This part-time position focuses on building relationships and providing technical expertise. Due to the local nature of this work we are seeking candidates located in Louisiana.

Group Care Organizational Development Consultant

The Centering Consultant is both expert in the clinical implementation of the Centering model and a skilled facilitation trainer. They are equally comfortable engaging with clinical staff and leadership to ensure that the systems changes required to meet CHI’s quality and sustainability expectations are achieved. They are skilled facilitators with the ability to transfer those skills to providers and clinical care teams tasked with providing Centering care.

Practice Services Manager

The Practice Services Manager (PSM) manages the client relationship through its lifecycle and is the primary point of contact for all Centering practice sites within a geographic region. The PSM assesses site needs, coordinates the delivery of products and services necessary to implement Centering group care as well as provides regular outreach and support to ensure that sites are reaching their goals. The position is customer-service driven with a focus on anticipating site needs and identifying solutions to common challenges.

Practice Services Coordinator

The Practice Services Coordinator (PSC) is an integral member of the Practice Services team and reports to a Practice Services Manager. The PSC is responsible for coordinating activities and operations that support Centering practice site implementation and customer service needs through contract support, event management, and vendor relations. This individual ensures excellent customer satisfaction through detailed planning, organization, and clear and responsive communication.

New Jersey Centering Program Manager

The NJ Centering Program Manager (CPM) position supports the expansion and sustainability of Centering at healthcare practices within New Jersey, with a special focus on the Healthy Women Healthy Families (HWHF) Initiative through the NJ Department of Health (DOH). The CPM provides direct technical assistance and training to current Centering practice sites and helps to identify and recruit new sites to implement Centering. The position also requires engagement with the DOH, community partners, and payers.
