Our team provides implementation support to guide healthcare practices through every step of the system redesign and to build a foundation for a successful, sustainable practice. We have developed curricula and patient materials, and our facilitation training offers a variety of skill-building and interactive learning activities that prepare facilitators to lead engaging groups. We offer practice management and support tools, and site accreditation for model fidelity and quality assurance. We are engaged in advocacy efforts for payment reform and community outreach to achieve the Quadruple Aim of better health, better care, lower cost and provider satisfaction. About Centering®

Centering® is an innovative, evidence-based model of group care that brings patients out of the exam room and into a comfortable group setting. Clinicians see a group of 6-12 patients with similar health conditions who receive the highest quality of care and, as part of an ongoing group, form a supportive community where they gain knowledge, skills and confidence. The Centering model utilizes a facilitative process that incorporates health assessment, interactive learning and community building to help support positive health behaviors and drive better health outcomes. With over two decades of experience working closely with healthcare providers from all sectors, CHI has developed and sustained the Centering group care model in more than 500 practice sites in the U.S.

The Position

The State Program Manager (SPM) is responsible for driving the expansion of the Centering model at healthcare practices within their state through exceptional customer service and engagement with key stakeholders in the provider and payer communities. This part-time position focuses on building relationships and providing technical expertise. The SPM is equally comfortable interacting with healthcare providers and administrators, payers, community stakeholders, and organizational leadership across a wide range of government, insurance and clinical practice settings. They can speak knowledgeably and authoritatively on the value of Centering to each constituent group and serve as a go-to resource for implementation support.

Supervision

State Program Manager is supervised by a Regional Director.

Location

  • Louisiana
  • Due to the local nature of this work we are seeking candidates located within the target state
  • Relocation funds are not available for this position
  • This employee will be working from a home office

Responsibilities

New Practice Engagement & Development

  • Identify and reach out to potential practices to educate leadership, administrators, and providers about the value of the Centering model
  • Work in close collaboration with Centering sites to identify state funders and funding opportunities to help support the startup costs of implementation and training
  • Work collaboratively with the Regional Director to identify state opportunities and problem solve challenges

Centering Technical Assistance / Implementation Support

  • Provide consultation and technical assistance to practice sites on system change, and Centering practice sustainability

Payer and Stakeholder Development

  • Engage with key stakeholders such as, healthcare providers and administrators, payers, community stakeholders, and organizational leadership across a wide range of government, insurance and clinical practice settings
  • Educate stakeholders about the value of Centering, how it helps providers and payers meet key quality measures, how Centering fits into alternative payment models, and what payers in other states are doing

Administration & Other Activities

  • Meet regularly with the Regional Director and other SPMs within the region to problem solve, report and plan activities and remain current with CHI initiatives

Travel

  • The SPM spends a great deal of time in the field meeting with potential practices, supporting practices that are implementing Centering, meeting with payers and community stakeholders, and attending community events
  • Occasional travel to the Boston office may also be necessary

Experience, Skills & Attributes

  • Passionate about improving the U.S healthcare delivery system, in particular the patient experience of healthcare, health outcomes, and health equity
  • Highly motivated self-starter with an entrepreneurial spirit
  • Intelligent and articulate, skilled at presenting ideas and concepts
  • Comfortable speaking with organizational leaders, physicians, government agency heads
  • Minimum five to seven years health system experience or comparable
  • Proven skills in leadership, relationship development, collaboration and team building
  • Strong organizational skills, including the ability to manage multiple tasks at once, respond to tight timelines, pay attention to detail, and operate effectively in a fast-paced environment
  • Ability to work with diverse groups of people
  • Excellent communication skills (listening, speaking and writing)
  • Access to a personal vehicle
  • Bachelor’s degree, postgraduate degree a plus but not required.
  • Direct experience with Centering a plus
  • Familiarity with the clinical setting and healthcare payment infrastructure a plus
  • Bilingual/bicultural background highly desirable
