Is Your Practice Ready For Centering?

We are delighted that you are considering the Centering model of group care for your clinical practice. You may have a lot of questions about getting started and what you’ll need to successfully implement Centering. Keep reading below and watch this video to learn more.

The Readiness Assessment is the first step to initiating your relationship with CHI and beginning your implementation journey. It helps to identify some key characteristics of your clinical practice that may determine if Centering is right for you.

Once you have completed and submitted the Readiness Assessment your Practice Services Manager will contact you to schedule a telephone conversation to review your responses and further discuss what Centering may look like in your clinical setting.

Completing the Readiness Assessment will take approximately 30 minutes. You can click through the form to see what information is required. You can also save and return to the form later if you need to gather more data. It is helpful to have the following preliminary information collected before you get started:

  • Information on your patient volume (e.g. number of new OBs seen, number of newborns seen)
  • Number of providers who will engage with Centering
  • Information about potential group space (size estimate, availability of space for Centering groups)
  • Administrative support
  • Race, ethnicity and insurance mix of clinical patient population
