Request for Appli­ca­tions 2021

The Centering Healthcare Institute (CHI) has launched an aggressive plan to expand access to the Centering model of group healthcare in communities where group care can have the greatest impact. With this expansion, CHI has developed a program offering implementation support and facilitation training to 50 eligible sites - primarily in Federally Qualified Health Centers (FQHC) and Community Health Centers - eliminating the financial barrier to those in resource-limited communities.


Implementation Support & Facilitation Training (In-Kind Services)

If you are considering starting Centering or expanding your Centering practice to a new location, we can help. Valued between $21,000 - $54,000 per site, the award is designed to eliminate the financial barrier and bring Centering to communities where it can have the greatest impact.

Awardees are expected to begin Centering groups in the first year of implementation and have sufficient patient volume to start at least one new group each month. If your practice cannot meet these minimum patient numbers from your pool of patients identified for Centering, please carefully reconsider applying: In low-volume practices, Centering will need to be operationalized as the standard of care from day one in order to sustain the transition group care.

Please note that this is not a monetary award, but the awarding of in-kind services.


How to Apply

The Request for Applications outlines in detail the services awarded and requirements to apply.

Step 1: Connect with your CHI regional representative for assistance submitting a Readiness Assessment. If you are unsure who your representative is, please contact us to be connected.

Step 2: Based on the results of the Readiness Assessment, you may be eligible to apply for an Implementation Award. You will receive a link to the online application narrative from your CHI regional representative.

Applications are comprised of five components:

  1. Application Narrative
  2. Sustainability Budget
  3. Commitment to the conditions of the project Memorandum of Understanding (MOU)
  4. Letter of Support from practice leadership indicating a commitment to an on-going budget for Centering, scale-up to sustainable patient volume over time, release time for Centering coordination and data entry and buy-in from the C-Suite and front-line staff
  5. Commitment to use the CenteringCounts online data system which will require a Business Associate Agreement (BAA)
  • Applications can only be submitted online
  • No hard copies will be accepted
  • We urge you to write and save your narrative answers offline in Word or another program first. Once final, paste content into the narrative online to avoid losing all of your work if your browser crashes during submission.


The goals of this expansion are to increase patient access to Centering care and increase our reach from 70,000 to 300,000+ patients per year. Based on past research, CHI expects the spread of Centering models to demonstrate a profound impact on health outcomes and equity at the selected sites:

CenteringParenting Goals:

  • Increased safe sleep practices
  • Extended breastfeeding
  • Increase rates of current immunizations
  • More developmental screenings conducted
  • More access to oral health services
  • More moms with healthy BMI
  • More screenings for intimate partner violence & postpartum depression

CenteringPregnancy Goals:

  • Preterm birth at least 1/3 lower than local baselines
  • Reduction of black/white preterm birth disparity
  • Breastfeeding initiation rates improved by at least 10% vs. baseline

CenteringHealthcare Goals:

CenteringHealthcare is a framework that provides structure and resources to apply the Centering methodology to any patient population appropriate for group care, goals will vary based on the patient population and local baselines.

Timetable for Applications

Applications will be reviewed on a rolling basis throughout 2020 until all slots have been awarded. You can expect to hear back within four months of applying.
