Join Laura Wise, MD and Zalak Trivedi, CDE, who will share the curriculum, logistics and select interactive activities in their Spanish speaking Centering Diabetes group in an FQHC setting. Their discussion will include co-facilitators roles; the major benefits of the group model including: building social support, improved diabetes care, improved metrics; and logistical challenges of staffing needs, recruitment & retention.

Laura Wise, MD, MPH is a family physician and group visit facilitator who has developed group curricula and led Spanish and English language group visits in FQHC clinic settings for 20 years.Her experience also includes training other healthcare providers in group visit facilitation including CenteringParenting and other group models.Her passions are finding ways to help patients have a voice in their care, and creating opportunities for patients to build social support in the healthcare setting.

Zalak Trivedi, MS, RDN, CDE is a Registered Dietitian (RD) and a Certified Diabetes Educator (CDE) and has been counseling patients to manage chronic diseases in both individual and group settings at an FQHC clinic. In her work, she is guided by the principles of Medical Nutrition Therapy, Food Is Medicine and Motivational Interviewing. Zalak has cocreated and currently cofacilitates several Group Medical Visits at Hayward Wellness Center.
